What you will do
- Undertake statutory reviews of care and support plans in accordance with the Care Act 2014, ensuring services remain appropriate and outcome-focused.
- Complete social care assessments for adults and carers, including reassessment of needs where circumstances have changed.
- Carry out Mental Capacity Assessments and contribute to Best Interests decisions in line with the Mental Capacity Act 2005.
- Identify and respond to safeguarding concerns, progressing issues appropriately in line with local procedures.
- Develop and review care and support plans, ensuring a strengths‑based and person‑centred approach.
Person Specification
Qualifications
- Hold a degree in Social Work or an equivalent recognised qualification.
- Registered with Social Work England.
Registration & Compliance
- Possess knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
- Understand CQC standards and local authority statutory duties.
- Enhanced DBS clearance is required, ensuring you meet the highest standards of professional practice.
