Role Description
We are recruiting experienced permanent full‑time and part‑time Social Workers to work across Lancashire as part of our Older People & Physical Disabilities Service. Responsibilities:
- Undertake strength‑based, outcome‑focussed assessments and support planning, ensuring that residents receive the right amount of support.
- Support people to access local services, maintain independence and promote health and wellbeing.
- Work alongside other agencies, professionals and informal carers to provide a holistic service.
- Co‑operate with supportive managers and participate in various training and progression opportunities.
