Salary £36,363-£40,777 per annum. Permanent position. Service: Older People & Physical Disabilities. Adult Social Care. Location: East Lancashire.

Role Description

We are recruiting experienced permanent full‑time and part‑time Social Workers to work across Lancashire as part of our Older People & Physical Disabilities Service. Responsibilities:

  • Undertake strength‑based, outcome‑focussed assessments and support planning, ensuring that residents receive the right amount of support.
  • Support people to access local services, maintain independence and promote health and wellbeing.
  • Work alongside other agencies, professionals and informal carers to provide a holistic service.
  • Co‑operate with supportive managers and participate in various training and progression opportunities.
