Warwickshire County Council is pleased to announce due to expansion of the service, we are looking for one full time Reablement Co-ordinator to join Warwickshire’s Reablement team. We are a well-established team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, Reablement Assistants and Social Care Practitioners, who have provided years of interventions for Warwickshire Reablement people and aim to achieve the best outcomes going through their Reablement journey.
Reablement focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. We do this by supporting people to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home.
We believe that by supporting people to identify and achieve their personal goals and outcomes, and by embedding personalisation, people will have choice and control over how their support will be delivered, this will have a positive impact on health and well‑being.
The Role
As a Co-ordinator, your role will include line management of Warwickshire County Councils Reablement Assistants. You will ensure a safe and effective Reablement service is offered to the people of Warwickshire. Work in partnership with the Reablement Assessment Team and other professionals to provide an outcome focused service to people that promotes independence and maximises daily living skills.
Undertake, develop and manage work programmes for Reablement Assistants, with the aim of providing a consistent service that is safe, high in quality and cost effective. You will manage the Reablement referral through their adult journey within Reablement. You will also ensure that needs and outcomes identified within the Reablement plan are met.
The posts will be Countywide but you will be based at Kings House, Bedworth. The post will be for 37 hours per week. There will be an expectation that the post holder will be required to provide countywide support to Warwickshire Reablement service therefore means to travel across the county is essential.
Key Requirements
- Dedicated individuals with a passion for care provision, making a positive difference for the people living in their communities.
- Respond preventatively to referrals for social care and support within the service.
- Manage a small group of Reablement Assistants.
- Manage your time and workload and work closely with colleagues within the team and external agencies, as well as independently.
- Excellent communication skills and ability to work within a team and on your own initiative.
- Good time management and organisation skills; solutions‑focused, flexible, and willing to travel across the county as needed.
- Handle confidential information with tact and discretion.
- Work shift pattern: 7 am‑3 pm, 9 am‑5 pm and 2 pm‑10 pm Monday to Friday, one weekend in every three weeks, and bank holidays on a rota basis.
