The Role

£25,152 to £27,919 + 7% enhancement for out of hours working and essential car user allowance up to £963 pa. pay award pending

The role of Home Support co-ordinator will include the supervision of a dispersed team of Home Support Workers who work across the borough providing reablement services to people within their own homes and working with a range of internal and external partners to achieve the best possible outcomes for individuals. With the aim of supporting individuals to retain, regain and develop the skills to manage their lives and environment. The role will include the safe selection and recruitment of workers and their retention in care services. Allocate and monitor the progress and quality of work in your area of responsibility. Assist and contribute to the development of risk management plans to support individuals’ independence within their home, contribute to care plans and their review and the protection of individuals from harm and abuse.

About You

You must be a motivated and compassionate person with experience in health or social care and confidence in supervising staff and coordinating services. You will communicate effectively, manage priorities, and work well with colleagues, partners and carers. You will understand relevant legislation, be confident using IT systems, and make sound decisions in a fast-paced environment. You must be physically able to meet the demands of the role and flexible to work on a 5from7 rota, including evenings, weekends and bank holidays, to meet service needs.
