Health Support Worker

We are looking to recruit a health support worker to join the West Older Peoples Community Mental Health Team working from Poplar house in Armley. The successful candidate will work as part of a multi‑disciplinary team, working closely with other community and inpatient mental health services. As a Health Support Worker you will deliver a range of care interventions, providing a high standard of care for Older People within a community setting. This will include delivering care in line with agreed care plans under the supervision of a qualified member of staff. You will be required to work independently with service users, for which you will receive regular supervision. The successful candidate will also be supported in their professional development.

The Older Peoples Community Mental Health Team provides care and treatment Mon‑Fri 9-5, 5 days a week.

Main duties of the job

As a Health Support Worker, you will be expected to work with patients, carers and the Multi‑Disciplinary Team to provide care to people with mental health needs and/or dementia within their homes. Although working under supervision you will be implementing interventions on your own. Being passionate about this age group and demonstrating a calm and confident approach is essential. Experience in mental health and dementia would be desirable but not essential as you will be working under supervision and training is available.

You will have excellent communication skills and will show that you have the ability to form and sustain therapeutic relationships with people with mental health needs and/or dementia.

The role requires the candidate to support the care, assessment, interventions and activities for service users mainly in their own homes and environments.

Being a car driver and access to a car is essential.

Person Specification

Experience

  • Experience of working with a range of older people with complex mental health needs, dementia and/or frailty in a hospital inpatient or a community mental health setting
  • Experience of working in the community within a mental health setting

Knowledge

  • Knowledge and experience of supporting service users with planned psychosocial interventions

Skills

  • Holds a UK driving licence and has access to a car
  • Demonstrate a desire to work with older adults
