We are recruiting a Band 3 Mental Health Support Worker to join a newly established Community Rehabilitation Team (CRT) within Mid and North Hampshire Divison.
This is an exciting opportunity to be part of a new, community-based rehabilitation service, supporting people with complex mental health needs to live independently, develop their recovery and maintain stability within the community.
You will provide direct, person-centred support to individuals, working alongside a multidisciplinary team to help people build skills, confidence and social networks.
The service focuses on helping people to:
- Maintain their independence
- Develop daily living skills
- Build confidence and social inclusion
- Sustain living in their own home or supported accommodation
The team works using an assertive outreach model, delivering care in people’s homes and community settings.
As a Mental Health Support Worker, you will:
- Work directly with service users to support recovery and wellbeing
- Deliver structured interventions and activities as part of care plans
- Support individuals to manage their social and practical needs
- Assist practitioners in assessment, care planning and review processes
- Build supportive relationships to encourage engagement and independence
- Contribute to a safe, responsive and recovery-focused service
This is a community-based role, involving travel, lone working and supporting people in a range of settings.
