Role Overview
This is a permanent role on a part‑time basis of 22.5 hours per week. Shifts are from 8 am – 4 pm or 12 pm – 8 pm, covering all seven days of the week.
The position is for a Registered Community Nurse who will care for patients in their own individualized environment, implementing patient‑centred care.
Key Responsibilities
- Caring for patients in the community setting.
- Collaborating with Integrated Community Team colleagues.
- Managing a small caseload of patients in partnership with line manager.
- Participating in individualised development planning.
Qualifications
- Current NMC registration.
- Post‑registration experience is helpful but not essential.
- Ability to work a shift system that includes early and late shifts across 7 days, including weekends and bank holidays.
