Hub Coordinator

Hours available: up to 60 hours available (hours to be agreed on appointment)
The Hub will act as a single point of access for all referrals, enquiries and contacts related to Community Therapy/Rehab Centre and District Nursing and other community services, making it easier for patients and professionals to connect with the right care quickly and efficiently.
We are seeking enthusiastic, adaptable individuals to join us in developing and delivering this innovative service. You will be part of a collaborative team dedicated to creating a seamless and supportive pathway for our community services.

Job responsibilities
This position requires you to manage the Hub’s operations, ensuring smooth coordination of referrals and communication between services, maintaining accurate records, and providing customer support to patients and staff. Person Specification
Qualifications
- GCSE (A–C) (9–4) in Maths and English or equivalent
- Level 2 or 3 certificates or diplomas in Business Administration or Customer Service
- Business Administration Apprenticeship (optional)
Knowledge and Experience
- Demonstrable experience in an administrative role
- Experience working with electronic management systems
- Experience using Microsoft Office programmes
- Ability to work independently without direct supervision
- Experience in customer service, answering phone calls and communicating with a range of stakeholders
- Experience with data protection and privacy as per GDPR
- Experience in health or social care environments
- Experience using NHS electronic systems such as EMIS, ICE, e-Community, STRATA
Skills and Aptitudes
- Excellent telephone skills and clear customer service skills
- Friendly, tactful, articulate, sympathetic and self‑motivated
- Ability to adjust to changing priorities and tasks while resolving issues efficiently
- Ability to prioritise tasks and deadlines ensuring accuracy and completeness

Other Requirements

Disclosure and Barring Service Check
This post requires Disclosure and Barring Service (DBS) and is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. DBS charges are deducted from salary over 4 months or as a one-off payment:
- Standard Check: £28.00
- Enhanced Check: £56.00

Pay
£25,760 to £27,476 per annum (pro rata)
