Urgent Care Home Support Worker

HCRG Care Group is looking for a motivated Support Worker to join the Urgent Care Response and Intermediate Care Team in Sheerness. This full-time role at 37.5 hours per week focuses on supporting patients in their homes post-illness or injury, preventing unnecessary hospital admissions, and ensuring successful discharges.

The ideal candidate will have NVQ Level 2 in Health and Social Care, with experience in healthcare settings and strong communication skills. The position also requires flexible weekend work.
