*Provider Support Assessor – Home First Community – #000B27691

Role Overview

Sheffield City Council is recruiting four full‑time Provider Support Assessors for the Home First Community service (formerly the Short Term Intervention Team), based at Manor Centre, Sheffield. This city‑wide, field‑based role focuses on hospital discharge support and short‑term community care for older adults aged 65 and over.

Responsibilities

• Work Monday to Friday between 08:00 and 20:00 (8.5‑hour shifts with a paid break).
• Manage daily referrals via the LASS (Liquidlogic) system.
• Conduct initial assessments, mobility and environmental risk assessments, and review visits.
• Develop support plans and undertake medication checks.
• Maintain accurate records and manage individual caseloads.

Qualifications & Experience

• Experience in social care, community nursing, domiciliary care coordination, or support work – experience is prioritised over formal qualifications.
• Level 2 or 3 qualifications are not essential.

Requirements

• Full UK driving licence and access to a private vehicle, with willingness to travel citywide.
• Resilient, confident, highly organised, and able to work autonomously in a fast‑paced environment.
• Enhanced DBS required.

Compensation & Contract

• £17.18 per hour, paid weekly.
• Contract from 29 June 2026 to 28 December 2026.
