Care Leaver Transition Support Worker – Staying Close

Tameside Council is seeking a dedicated Transition Support Worker to assist young people exiting care, developing their independence skills.

This role involves working closely with young individuals to provide practical, emotional, and social support necessary for their transition to independent living. The ideal candidate will have a passion for helping others and the ability to foster positive relationships.

Work will be based out of Ashton Old Library, and this is a fixed-term contract until March 31, 2027, with a salary range of £31,022 to £33,699.
