Apprenticeship – Development Post

We are seeking a Rehabilitation Support Worker (RSW) to join our Neighbourhood Rehabilitation Team. This is a development post with apprenticeship support to achieve a Level 2 qualification in Health & Social Care.

Job Summary

The RSW will work under the guidance of registered healthcare professionals to provide high quality, person‑centred care on the Intermediate Care pathway, promoting independence and participation in physical exercise programmes.

Responsibilities

  • Work independently and collaboratively supporting individuals in their own homes and care homes.
  • Collaborate closely with the Multi‑Disciplinary Team (MDT); excellent communication and time‑management skills are essential.
  • Assist in the initial assessment and development/implementation of care plans.
  • Participate in training, education, and clinical supervision to develop skills and knowledge, applying continuous reflection on own practice.
  • Maintain accurate clinical records.
  • Support safe use of equipment such as hoists, walking aids, and pressure cushions.
  • Use a vehicle for travel related to the duties of this role; access to a car is essential.
  • Work a shift pattern that includes weekends and bank holidays; flexible rostering for 7‑day care may be required.
  • Respond to verbal or physical aggression or challenging behaviour, recognising and reporting changes, and ensuring personal safety while supporting service users.
  • Assist individuals who exhibit challenging behaviour or present a significant imminent risk.
  • Identify and manage risks, including moving and handling assessments, using local policies and procedures.
  • Contribute to therapeutic relationships by adopting a polite, positive, and empathetic approach.
  • Follow policies, procedures, protocols, guidelines, and established clinical care plans.

Training & Qualifications – Essential

  • Complete all Trust mandatory training, including the Care Certificate and requisite refresher training.
  • Undergo development and training, including, if eligible, a Level 2 qualification in health and social care.
  • Hold GCSE grades 1‑9 (certificate check) or functional skills level 1 or equivalent.
  • Be aged 18 or older and a UK resident for at least three years (student visa excluded).

Training & Qualifications – Desirable

  • Prior experience in a caring environment or voluntary role.
  • Level 2 health and social care qualification.
  • Additional language skills other than English.
