A well-respected Local Authority in the Caerphilly area is seeking an experienced Qualified Social Worker to join their Community Team. This is an excellent opportunity for a skilled practitioner looking to take on a varied caseload within a supportive and established service.

Key Responsibilities

• Undertake strengths‑based social care assessments
• Manage a diverse community‑based caseload
• Arrange and review Packages of Care
• Coordinate Direct Payments
• Support placements in Extra Care Housing, Supported Living, and Residential/Nursing homes
• Work collaboratively with multi‑disciplinary professionals and partner agencies
