Job Overview

The LD CRIST is a multidisciplinary team of learning disability & mental health nurses, health care support workers, psychologists, speech and language therapists, occupational therapists and psychiatrists who provide a service 7 days a week from 08:00-21:00, including bank holidays. The team aims to prevent unnecessary hospital admissions by providing rapid assessment and intensive support to people with learning disabilities, challenging behaviours or mental health conditions.

Opportunities have arisen for Band 3 Community Learning Disability Support Workers to join LPT to support people and their families in a community setting.

Job Description Summary (Main Duties and Responsibilities)

  • Contribute to assessments and interventions, including direct observations, risk assessment and care planning.
  • Support clients in maintaining health and wellbeing.
  • Undertake specified and delegated clinical and therapeutic activities.
  • Assist the healthcare team under supervision with planning, organising and provision of clinical care.
  • Maintain high standards in a safe environment that promotes equality and sensitivity for all individuals.
  • Assist registered professionals with implementation and evaluation of individualised care plans.
  • Accurately record patient information and provide support and supervision to colleagues and learners as appropriate.

Candidate Requirements

The ideal candidate will have previous experience working with adults or children with learning disabilities in a community setting and possess excellent communication and interpersonal skills with the ability to work independently and collaboratively.

Additional Requirements

A full UK driving licence and access to a vehicle for use during working hours are required.
