What you will do

  • Undertake Care Act 2014 assessments, focusing on strengths-based and outcome-focused evaluations of individual needs.
  • Develop and review personalised care and support plans, ensuring they are well-resourced and effective for the service users.
  • Manage a diverse caseload by prioritising risk and complexity, contributing to the flow and effectiveness of service delivery.
  • Lead safeguarding enquiries under Section 42 of the Care Act, actively working to protect adults at risk.
  • Foster collaborative relationships with NHS partners, housing services, providers, and voluntary sector organisations.

Person Specification

Qualifications:

  • Hold a degree or equivalent qualification in Social Work.
  • Completion of the Assessed and Supported Year in Employment (ASYE) is advantageous.

Registration & Compliance

  • Be registered with Social Work England.
  • Possess an in-depth understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks.
  • Demonstrate awareness of the Equality Act 2010 along with relevant local authority responsibilities.
