CHC Assessments: Adults’ Social Worker (Hybrid)

Community Care Live is seeking an Adults’ Social Worker to join a Healthcare Assessments team in the UK. This role requires a Social Work Qualification and a minimum of 2 years post-qualified experience.

You will work with disabled adults, assessing their needs and utilizing the Decision Tool Support (DST) while travelling countywide. The position offers hybrid working, excellent training opportunities, and a pay rate of £30.00 per hour.
