Adult’s Social Worker needed for our Community Learning Disabilities Team in Havering, paying £30 per hour

Are you passionate about empowering adults with learning disabilities to live fulfilling, independent lives? Havering is looking for a dedicated Social Worker to join our Community Learning Disability Team, where you’ll play a vital role in delivering personalized, strengths-based support that helps people achieve the outcomes that matter most to them.

Working as part of an integrated multi-disciplinary team, you’ll collaborate with health professionals, community partners, service users, and carers to provide high-quality, person-centred social work practice.

Key Responsibilities

• Manage a varied caseload, including complex cases where appropriate.
• Complete person-centred assessments, reviews, and care planning.
• Undertake safeguarding enquiries in line with statutory responsibilities.
• Support safe hospital discharges and help prevent unnecessary admissions.
• Develop and review personalized support plans that promote independence and wellbeing.
• Work collaboratively within a multi-disciplinary team and alongside partner agencies.
• Maintain accurate, timely, and high-quality case records.
• Contribute to service development and, where appropriate, provide supervision and support to colleagues.
