Social Worker – Preparing for Adulthood

Liquid Personnel is recruiting a Social Worker for the Preparing for Adulthood Team for its client in Bury.

Responsibilities

• Supporting transitions planning – working with young people (14–25) to develop person‑centred transition plans that promote independence, wellbeing and long‑term outcomes.
• Completing Care Act assessments – assessing eligible needs as young people move into adulthood, creating strengths‑based support plans and coordinating appropriate adult services.
• Multi‑agency coordination – collaborating with education, health, SEND teams and families to ensure smooth transitions and consistent support across children’s and adults’ services.

Qualifications & Experience

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post‑qualified experience
• Degree level or equivalent in Social Work
