Liquid Personnel is recruiting a Social Worker for the Preparing for Adulthood Team for its client in Bury.
Responsibilities
- Supporting transitions planning – working with young people (14–25) to develop person‑centred transition plans that promote independence, wellbeing and long‑term outcomes.
- Completing Care Act assessments – assessing eligible needs as young people move into adulthood, creating strengths‑based support plans and coordinating appropriate adult services.
- Multi‑agency coordination – collaborating with education, health, SEND teams and families to ensure smooth transitions and consistent support across children’s and adults’ services.

Qualifications & Experience
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post‑qualified experience
- Degree level or equivalent in Social Work
