An exciting opportunity has arisen for an enthusiastic, organised, highly motivated, and experienced Registered Nurse to work as part of the Community Nursing Triage Team within Wokingham Hospital, Wokingham. Your role as a Triage nurse will be to manage referrals via the telephone from patients, GP practices and other service users. Therefore, you will be required to have excellent communication and problem‑solving skills and will be self‑motivated, forward thinking, and autonomous.
To keep your clinical skills updated you will be required to work within one of the community nursing teams, working alongside other clinical staff in order to provide a high quality, locally based service for the patients within the Wokingham Locality. Therefore, the post holder must be adaptable in their approach to work and have access to a car and a valid UK driving licence.
Main duties of the job
- Provide clinical support, assistance, and professional expertise to all callers to the Community Nursing Triage Telephone Support. This will range from signposting the referral to the appropriate service, making a direct referral, or offering advice.
- Work in conjunction with administrative support staff to provide high quality, clinical triage using knowledge, skills, critical thinking and professional judgement.
- Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
- Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
- A key member of the Community Team, providing robust clinical services for patients, maintaining positive communication links with the GP practices and other members of the Multi‑Disciplinary Team, providing high quality patient‑focused care using evidence‑based practice.
- Your working hours would be mostly Mon to Fri, 8.30am to 4.30pm (with approximately one weekend every 4‑5 weeks and bank holidays in rotation). The Community Nurse service will also be covering the 4:30 to 19:00pm shift on a rota basis.

Education/Qualifications/Training
- Registered Nurse.
- Current NMC Registration.
- IT knowledge and accurate keyboard skills.
- 2 years physical clinical experience within a Community setting.
