Do you fancy trying a new approach to working with older people within a hospital acute setting? The Hospital Social Care team is currently pioneering a new approach to front‑line working which focuses on customer strengths and promotes the use of universal services. We have three hospital sites across Warwickshire County Council/Coventry. We are dedicated and supportive teams, aiming to respond quickly and efficiently in offering strengths‑based assessments and providing support to customers and their carers within a fast‑paced environment.
The teams have a mixture of staff skill sets and roles that include: Social Workers, Social Care Workers, Admin, Trusted Assessors, Team Managers & Senior Managers.
About the Role
Joining our established Hospital Team, you will provide a social work service to a highly professional standard. Managing a caseload and travelling throughout North and South Warwickshire depending on business needs, you will work with older adults, their families and unpaid carers. Undertaking and recording robust assessments and care and support planning will also be part of your role.
Additionally, you will communicate with a wide variety of customers, communities, health partners and agencies, working independently and often under pressure. Team members receive regular supervision and day‑to‑day support from colleagues and Team Managers, and access to regular training provided by Warwickshire County Council.

Key Requirements
- Professional Social Work qualification and registration with the Health and Care Professions Council.
- Ability to work 5 days over 7 days, supporting a streamlined service and timely discharges. The hospital teams work one weekend a month (four‑week rota); candidates should be prepared to work weekends.
- Work closely with the Multidisciplinary Health & Social Care team, participating fully in duties, team meetings, and other meetings determined by the Team Manager and/or Service Manager.
- Driver’s licence essential for the role, as you will be required to complete post‑discharge reviews, especially in care homes.
- Dynamic approach and willingness to embrace the new strengths‑based way of working.
- Decision‑making ability and capacity to meet tight deadlines.
- Adult social care experience and good information technology (IT) skills.
- Desirability of hospital social care experience – practitioners should be willing to learn within a fast‑paced environment.
- Experience using the Mosaic IT System in Warwickshire.
Social Worker Level 2
Level 2 denotes competence to plan, develop, monitor and evaluate outcomes within a healthcare setting.
