Position Overview

Directorate: Place Health & Adult Social Care | Service: Adult Social Care & Health

Salary: £35,412 – £46,142 (Career Graded)

Working Arrangements: Hybrid Working

Hours per week: 37

Contract type: Permanent

Language requirement: English to an appropriate standard.

Eligibility: Enhanced DBS Check required. Overseas residents may need an overseas criminal records check. The Council does not sponsor visas.

Key Responsibilities

  • Provide social work support to young people aged 14–25, across diverse settings.
  • Deliver assessment and support planning, applying legislation from the Care Act 2014, Children Act 1989, Mental Capacity Act and Human Rights.
  • Work within the Preparation for Adulthood Team to embed the PFA model and promote independence and life opportunities.
  • Apply strengths‑based, person‑centred practice and develop confidence and skills in service users.
  • Collaborate with a multidisciplinary team of Social Care Practitioners, patients, families and external partners.
  • Document and review progress, ensuring compliance with national guidance and local policy.
  • Support staff development and contribute to team improvement initiatives.

Qualifications and Experience

  • Good knowledge of legislation relating to adult and children’s social care.
  • Experience or passion for working with young people with various needs.
  • Understanding of the Care Act, Children Act, Mental Capacity Act and Human Rights.
  • Strong social work values and commitment to a strengths‑based approach.
  • Willingness to learn, develop and work as part of a new team under guidance of the Team Manager.
  • Effective communication skills and ability to engage with diverse populations.
