With previous experience from the NHS or private care, you can join the multidisciplinary team on a part time basis for 22.5 hours at Aderyn which has a strong emphasis on teamwork and offers a high quality step down service for men with enduring mental health conditions setting a sector‑leading example of clear care pathways for service users with a focus on recovery.
As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future.
You report to the Safeguarding Lead, and will manage internal investigations and liaise with local authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can see your career grow and you achieve your career aspirations.

Responsibilities
- Referrals to LADO and Local Authority in line with safeguarding and assessments
- Educating hospital staff about relevant safeguarding matters.
- Completing a full annual social care reassessment within the CPA format.
- Facilitating access to welfare rights, housing, advocacy services and associated support services.
- Participating in 1:1 supervision.
- Managing internal investigations and liaising with local authorities during admission and discharge.
- Managing social care assessments, risk assessments, group activities and contributing to ICR meetings.
Qualifications
- A Social Work degree.
- Registered with Social Work England or applied for.
- Experience within a similar environment.
- Experience with working with vulnerable adults.
- Background check (DBS) required.

You will be working at Aderyn, a high quality step down service for men with enduring mental health conditions. As well as a 17 bed rehabilitation unit there is also a 2 bed open rehabilitation cottage.
Individual and collaborative therapeutic timetables are developed for each patient and individual psychological therapies are offered.
