The role involves providing personalised, outcome‑focused care plans and supporting people across a range of settings, from community-based services and people’s own homes to Independent Hospitals and Adult Care Homes. Support Workers assist individuals with diverse needs, including learning disabilities, physical disabilities, mental health concerns, complex care, substance misuse, and more.
