Team Support Worker – Learning Disability Service

The Learning Disability Service in Dorset Healthcare NHS Foundation Trust is staffed by a passionate and dedicated team and is looking for a like‑minded individual to join us.
A Team Support Worker will provide care and support for patients and service users. You will be involved in implementing care plans and monitoring patients, reporting back to the relevant professional. You will be required to work in partnership with people with learning disabilities, their families and carers to support them to live successfully in the community.
Working part time (17 hours per week, days negotiable), the successful candidate will already have experience of working in a public‑facing setting. The role involves working within a multi‑disciplinary health and social care team to support people with learning disabilities to live successfully in the community.
Responsibilities
- Provide care and support for patients and service users.
- Implement care plans and monitor patients.
- Report back to the relevant professional.
- Work in partnership with people with learning disabilities, their families and carers.
- Support service users to live successfully in the community.
Qualifications
- Experience working in a public‑facing setting.
- Right to work in the UK; no visa sponsorship available.

Location
The post holder will be based at the North Dorset Local Office in Sturminster Newton but may be required to work across sites if a specific need arises.
