We are delighted to offer an exciting opportunity for a Generic Rehabilitation Support Worker to join our Integrated Community Stroke Service. In this rewarding role, you will work closely with therapists and nursing staff to support patients recovering from stroke.

Responsibilities

• Deliver personalised rehabilitation programmes for stroke patients.
• Support interventions across physiotherapy, occupational therapy, speech and language therapy, nursing care, and aspects of clinical psychology.
• Assist patients in the transition from hospital to home, optimising recovery and improving quality of life.
• Collaborate with a multidisciplinary team to provide high‑quality, community‑based care.

Qualifications

• Compassionate and motivated with a desire to develop skills within a supportive multidisciplinary team.
• Strong interpersonal skills and a commitment to patient‑centred care.
• Ability to work collaboratively with therapists, nurses and other healthcare professionals.
