We are looking for a Home Safe Support Worker to work within the Short‑Term Support Service. This post is fixed term – 13 months, part time 29.08 hours, £14.82‑£16.07 p/h.
The successful candidate will be part of a team providing short‑term limited interventions to promote people’s independence. The role typically involves visiting people in their own homes and supporting them to regain their ability to carry out a range of day‑to‑day tasks.
About us: The Home Safe Support team involves short‑term reablement care and equipment provision in service users’ own home to regain independence. You will work with individuals to set their personal goals and help them to achieve them.
The team consists of six Support Workers providing services seven days a week, 365 days a year. The role is to help individuals regain their previous level of independence at home following a hospital stay or short‑term discharge placement, and to support those who are unwell to help prevent hospital admission. Support may include assistance with personal care, help with medication, and assessing the need for low‑level equipment.
Our service is rated Outstanding by the Care Quality Commission.
Responsibilities
- Ensure that confidentiality is maintained, each service user is treated with dignity and respect.
- Risk assessments – develop and follow.
- Care plan – plan, implement and review, promote independence.
- Signpost onto other professionals or services.
- Medication – follow guidelines, policy and procedures, audits etc. Documentation – accurate and timely.
- Compliance with relevant legislation.
- Manage a caseload of service users.
- Competencies – therapy equipment.
- Training – relevant to the support worker post.
- Complete initial visits and home assessments.
- Interact and engage with colleagues.
Working Pattern
The working pattern is Monday to Sunday 8am to 8pm over a rolling rota (consisting of two shifts per day, either early or late).
Requirements
- Valid UK driving licence and access to a car for work purposes.
- Ability to talk easily with service users and provide advice in accurate spoken English.
- NVQ Level 3 / equivalent or willingness to work towards an equivalent qualification.
- Good general IT skills.
