Overview
We are currently looking for an Adult’s Social Worker to join a Hospital Discharge Team.
Team
This team supports adults who have been discharged from hospital, helping them return safely to the community with an appropriate care plan tailored to their individual needs. The team works with adults aged 18 and over who may have learning disabilities, mental health needs, and/or physical disabilities.
They undertake a range of assessments, including Discharge to Assess, Continuing Healthcare (CHC), and Care Act 2014 assessments.
The team is made up of Occupational Therapists and Social Workers who work collaboratively to ensure individuals receive the most appropriate support and care to promote their independence and wellbeing.
Role and Responsibilities
The successful candidate will assist in the assessment and planning of care for discharged adults, working closely with the multidisciplinary team to ensure continuity of care and promote independence.
Qualifications
- Social Work Qualification (Degree/DipSW/CQSW) with a minimum of two years post‑qualified experience.
- A valid UK driving licence and vehicle is preferred for this role but not essential.
