Somerset NHS Foundation Trust is seeking a dedicated individual to join the Connect18 team, a unique transitions service supporting young people moving from CAMHS to adulthood. The role involves building meaningful therapeutic relationships and providing tailored engagement strategies during crucial transitions.
Working hours are Monday to Friday, 9am–5pm, with travel across Somerset to meet young people, promoting flexible and responsive support. We encourage applications from experienced practitioners and newly qualified professionals registered with NMC or HCPC.
