Do you have experience of working as a Specialist Community Public Health Nurse (School Nursing) and the desire to facilitate and support the delivery of a high quality School Nursing service?

We have a vacancy for a Community Team Lead in the Public Health Nursing 5-19 service within the Dacorum and St Albans School Nursing Team. This is an ideal opening for those wishing to further their career and who have a desire to move into a leadership role within the School Nursing arena.

The 0-19 PHN service has recently been retained by HCT following competitive tender and is in a period of service mobilisation and development. This is an exciting opportunity to influence delivery of the service and empower staff through the change process

This is a permanent contract and there will be a robust support package available with professional development and training opportunities.

Main duties of the job

The successful candidate will:

Manage the day to day running of the team and offer support and guidance, including assessing staff coverage to minimise risk in covering the service

Identify areas that are not performing as well as expected and act on this to find out why and rectify

Appraisal and regular 1-1 meetings with staff that are direct reports

Ensure delivery of the Healthy Child Programme and the elements included within this

Ensure the set Key Performance Indicators (KPIs) are met

Analyse team performance data on a regular basis

Will work in partnership with the Family Support service and other partner agencies and professionals ensuring children and their families can access the same level of support

Work with the 3 other School Nurse Community Team Leads across the county to ensure a high quality, consistent 5-19 service

Be part of the 0-19 leadership team within Dacorum and St Albans to facilitate a seamless 0-19 Public Health Nursing service

You must hold a full valid driving license and access to a car, to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010) to be able to travel efficiently around the local geographical area. Provide evidence that the vehicle is insured for business use.

Detailed job description and main responsibilities

To lead and supervise the members of the Public Health nursing team in the delivery of the 5-19 Healthy Child Programme (HCP) and Public Health Nursing Programme in line with the Trust guidelines and public health priorities.

The post holder will work with Partners to identify population health needs and determine the level of need and support required. The post holder will represent the Public Health Nursing (PHN) 5-19 service at their area’s liaison meetings e.g. Family Support Service meetings, Pastoral Leads meetings in schools and other appropriate meetings and give feedback information as required.
