Are you an experienced Occupational Therapist looking to make a real difference in supporting people to recover safely at home?
We’re looking for a motivated Band 6 Occupational Therapist to join our Home First Service, helping patients transition safely from hospital to home through timely assessment, rehabilitation and person-centred care.
Working as part of a supportive multidisciplinary team, you’ll manage a varied and rewarding caseload, delivering specialist occupational therapy assessments and interventions that promote independence, prevent unnecessary hospital admissions and support effective discharge planning.
What you’ll be doing
- Deliver specialist Occupational Therapy assessments and interventions within the community.
- Support safe and timely hospital discharge through collaborative discharge planning.
- Manage a complex caseload independently while contributing to multidisciplinary decision-making.
- Work closely with acute hospitals, GPs, community teams and local authority colleagues to provide integrated care.
- Supervise and support Band 5 Occupational Therapists, students and support staff.
- Promote rehabilitation, independence and positive outcomes for patients with complex health and social care needs.
About you
You’ll be a HCPC-registered Occupational Therapist with experience managing complex patients and working collaboratively across multidisciplinary teams. You’ll have excellent assessment, communication and clinical decision-making skills, together with a passion for delivering high-quality, person-centred care within the community.

The Home First Service operates seven days a week between 8:00am and 8:00pm, including bank holidays. Full-time staff work flexibly across a seven-day rota, including early, late and long-day shifts.
