Location: Stonehouse Hospital
Hours: 35 hours per week, permanent
Salary: £37,702 – £38,779 per annum
Responsibilities
- Co‑ordinate active AWI cases, ensuring timely communication with families, carers and private law firms in line with AWI procedures and best practice
- Provide support to the Hospital Discharge Mental Health Officer
- Update and maintain records over multiple platforms such as Liquid Logic and Microsoft Excel
- Work with adults aged 16 years and older who have a range of health and social needs
- Complete holistic risk and needs assessment, develop care support plans, review existing plans and support duty work under the supervision of the Operations Manager
- Participate in the duty rota
Qualifications and Experience
- Highly motivated, organised, willing to learn
- Experience working with vulnerable groups in the community
- Willingness to be involved in statutory checks such as the PVG Scheme and, if applicable, an overseas criminal records check
