We are recruiting Band 5 Community Staff Nurses to join the District Nursing teams within CNWL Community Services in Hillingdon.

Key Responsibilitiesn
As a Community Staff Nurse, you will work both independently and within a multidisciplinary team to deliver safe, effective, and patient-centred care within the community.

You will develop skills including:

- n
- nAutonomous community nursing practicen
- nDiabetic management, blood glucose monitoring, and insulin administrationn
- nTissue viability and wound care, including:n
- n
- nCompression therapyn
- nDoppler assessmentsn
- nVAC dressingsn
- nPressure ulcer managementn
- nSurgical wound caren

n
- nBladder and bowel care, including catheterisation and continence assessmentsn
- nIntravenous medication administration and PICC/midline caren
- nPalliative and end-of-life care, including symptom management and syringe driversn
- nManagement of specialist drains including nephrostomies and chest drainsn

You will also:

- nMaintain accurate clinical documentation and patient recordsn
- nSupport patients, carers, and families with education and ongoing care needsn
- nWork collaboratively with specialist teams and wider community servicesn
- nContribute to safe discharge planning and hospital admission avoidancen

- nRegistered General Nurse (RGN) with current NMC registrationn
- nExcellent written and verbal communication skillsn
