We deliver high‑quality, safe services that improve the health, wellbeing and independence of the people we care for in the community, with the aim of avoiding unnecessary admissions into hospital.
Our Community Nursing Service provides high‑quality nursing care to patients in their own homes, operating 8am–8pm, seven days a week.
A full UK driving licence and access to a vehicle is required for this role.
Main duties of the job
- Work under supervision following written care plans and care pathways devised by the Registered Nurse.
- Follow prescribed care plans and treatment protocols.
- Follow HPCT Policies and Procedures.
- Have knowledge and skills to carry out prescribed tasks and report to the Registered Nurse any variances.
- Delegate responsibility for Patient Support Teams administrative tasks.
- Provide general administration support to the team.
Qualifications
- NVQ in Health care or equivalent, or willingness to commit to training.
- Full UK Driving Licence.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a Disclosure and Barring Service check.

Salary
£25,760 to £27,476 per year (pro rata for part‑time hours)
Working pattern
Full time or part time, depending on the location. Locations include Alton, Basingstoke, Bitterne, Southern Parishes, Hythe & Waterside.
