Senior Social Worker, D2A & Hospital Discharge (Band 6)

Midlands Partnership University NHS Foundation Trust is seeking an experienced Level 2 Band 6 Social Worker for the South Community Hospital Discharge teams in Lichfield. The role involves facilitating timely discharges and working with Multi-Disciplinary Teams.
