- Job Title: Hospital Social Worker
- Starting Salary: £38,220 – £41,771 per annum, plus £1,911 – £2,088 market enhancement pay, 5% operational allowance per year, and £1,000 welcome payment. Relocation and lease car terms apply – pro‑rata for part time. Pay award pending.

About the Role
You will help ensure safe and timely discharges from hospital by:
- Assessing the needs of patients and carers
- Arranging appropriate care packages
- Liaising with professionals and agencies
- Supporting person‑centred discharge planning
The role operates a 7‑day service and currently follows a 1‑weekend‑in‑8 rota across the Hospital Team. The majority of work is carried out through the Discharge to Assess model.
Requirements
- Social Work degree
- Registration with Social Work England
- Driving licence (ability to drive and/or travel around Hertfordshire)
- English fluency – the ability to converse at ease with members of the public and provide advice in accurate spoken English. A sign language interpreter may be required for those with a signed language requirement.
