Social Prescriber: Build Wellbeing & Community Connections

Mind (hfm) is seeking a social prescriber in York to improve community health and wellbeing. You’ll develop personalized action plans, foster connections with local services, and motivate individuals towards positive changes.

The role requires experience in working with complex mental health needs and the ability to build trust with clients. This fixed term contract will last until 31st January 2027, offering a salary of £27,200 per year.
