Senior Community Mental Health Practitioner Memory Assessment Service

Main duties of the job
- Provide thorough memory assessments for those referred into the service.
- Gather a detailed collaborative history of presenting problem from the patient and their carer.
- Collect relevant past medical history from system partners.
- Undertake physical health observations and be aware of escalation routes.
- Formulate a plan of treatment in collaboration with senior colleague in MDT.
- Assess risk and develop management plans, reviews and adjustments in line with the service user’s needs.
- Consider the holistic care needs and identify when and how to refer appropriately to other services.
- Work in collaboration with service users and their family/carers as experts in their own care.

Person Specification
Qualifications
- Registered Nurse.
Experience
- Registered Nurse, Mental Health Practitioner.
- Evidence of effective interpersonal skills.
- Able to use a computer & IT systems to meet the requirements of the role.
- Ability to work within a multi‑disciplinary team.
- Ability to act as a role model for junior team members.
Additional Criteria
- Experience of working in an OPMH setting.
- Experience of conducting formal memory assessments and using memory assessment tools.
- Knowledge/understanding of dementia and associated behaviours/interventions.
- Ability to organise self and others to achieve tasks.
- Knowledge of the key principles of safeguarding children and adults.
- Knowledge of the key principles of the Mental Capacity Act.
- Knowledge/awareness of the current NICE guidance relating to dementia and memory assessment pathway.
- Ability to facilitate clinical supervision.
