LDA Case Manager – Learning Disability & Autism (Band 7)

Black Country Healthcare NHS Foundation Trust in Wolverhampton is seeking a Learning Disability and Autism (LDA) Case Manager at NHS AfC Band 7 to lead complex caseloads and apply national guidance and best practice to improve outcomes.

You will work in a multi-disciplinary team, support the National LDA programme, and ensure timely reviews that reflect best practice and value for money within statutory frameworks.

Excellent communication and multi-agency collaboration are essential.
