We are seeking an experienced Learning Disabilities Assessment Manager to join our Liaison Care Transformation team, supporting the delivery of complex Continuing Healthcare (CHC) assessments for our client.
In this role, you will undertake complex Continuing Healthcare (CHC) assessments for individuals in receipt of CHC funding. You will review clinical records, assess ongoing support needs, and provide evidence‑based recommendations to ensure appropriate levels of care and funding. Working closely with commissioners, providers and Local Authority representatives, you will support the redesign and implementation of person‑centred care packages, develop clear KPIs for future monitoring, and produce robust clinical reports to support decision‑making, appeals and complaints. You will also ensure care plans are updated with a focus on meeting assessed needs and preventing avoidable hospital admissions.

Main duties of the job
- Conduct assessments, ensuring relevant paperwork is collected and shared with appropriate representatives prior to assessment, and undertake clinical assessments with individuals, representatives, providers (as required), and Local Authority representatives.
- Evaluate individuals’ needs based on medication records, care plans, daily living activities, and discussions with providers.
- Provide clinical advice and expertise to the assessment, care planning, and monitoring processes.
- Review cases to ensure they continue to be eligible and are structured according to need with appropriate levels of designated resources.
- Determine eligibility between Health and Social Care and make recommendations, taking into account the views of Local Authority representatives, on eligibility and appropriate level of support related to clinical and social care needs.
- Complete assessment documentation and ensure Local Authority comments are incorporated.
- Ensure care plans are updated based on assessment recommendations.
- Draft recommendations for ICB to consider, providing clear supporting evidence.
- Provide information and support to individuals and families through the assessment process.
- Provide evidence for all cases where there is a complaint.
- Collaborate with other professionals.
Person Specification
Qualifications
- Qualified to degree level in a related subject.
Experience
- Experience with Learning Disabilities CHC assessments.
- Experience in redesigning complex packages of care to meet clinical and social care needs.
Skills and Competencies
- Experience in Learning Disability assessments.

Depending on experience, day rates will be considered for interim contractor appointments.
