Job Description
As part of our outreach team, you will:
- Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes.
- Work closely with clients to implement and review tailored care plans, ensuring their safety and well‑being.
- Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma‑informed and recovery‑focused care.
- Maintain accurate records and report any changes in client condition or concerns promptly.
- Participate in ongoing training to develop skills in mental health support and person‑centred approaches.

- Experience supporting individuals with mental health conditions in community settings.
Our North & Mid Outreach team operates throughout the Burgess Hill, Haywards Heath, Crawley, and East Grinstead areas so being a car driver is essential for this role.
