Virtual Ward Care Flow Lead

Dougie Mac is seeking a proactive Flow Co-ordinator to support the HART Team in launching a Virtual Ward. The role focuses on day-to-day patient flow, referrals, admissions, and inter-service communication to ensure timely access to hospice care at home.

You will work with patients, families, and a multi-disciplinary team to monitor conditions, update records, and coordinate discharges while upholding confidentiality and quality standards.
