Clinical Support Nurse Specialist – Diabetes

An exciting opportunity has arisen for a band 6 nurse to join the diabetic specialist nursing team. These specialist nurses are based throughout the trust and provide specialist advice for all specialties within the Trust.

The successful candidate will undertake clinical assessments for patients and liaise with the wider MDT regarding ongoing care and treatment plans. We strive to deliver a high standard of care, ensuring safe and timely care to both patients and the MDT to manage acute and chronic conditions in an appropriate environment.

We are looking for enthusiastic and motivated individuals with an interest in diabetes. Specialist training and support from the MDT will be provided. Successful candidates must have at least 2 years qualified or have diabetes experience.

Job Responsibilities

  • The Nurse Specialist will provide support for the patient and family from diagnosis through treatment and throughout the diabetes journey, using advanced communication and psychological assessment skills to assess and provide management and education, and identify need for onward referral.
  • Provide education and specialist advice to patients, carers, and staff on specific care and treatment for the specialty, acting as a valuable resource for the multi‑professional team.
  • Set standards of care, influence the work of other professional groups and act as a change agent as a core member of the MDT, supporting the diabetes pathway to improve patient experience.
  • Lead the implementation of national diabetes strategy at a local level, including holistic needs assessments, patient‑focused education, training for self‑management, and promotion of person‑centred care.
  • Collaborate with the MDT to ensure prompt access to diagnostics and treatments, and actively participate in multi‑professional meetings as a patient advocate.
  • Contribute to service development, pursue excellence in care, and provide clinical specialist advice and support across the organisation.
  • Perform clinical assessments, plan and evaluate care interventions, manage symptoms, and reference professionals when appropriate.
  • Implement evidence‑based, individualised care and interpret complex clinical information to patients and families.
  • Run nurse‑led clinical services and, if a non‑medical prescriber, prescribe medication within expertise.

Qualifications, Specific Experience & Training

  • Registered General Nurse
  • First level degree or equivalent
  • Post‑Basic Qualification in Diabetes Care
  • Recognised Teaching qualification
  • Clinical diagnostics
  • Nurse Prescribing
  • Advanced Communication skills training

Knowledge and Skills

  • Demonstrable experience within diabetes/specialty
  • Evidence of continued professional development
  • IT Literate
  • Previous CNS experience
