Overview
Hull University NHS Teaching Hospitals (HUTH) are collaborating with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model for people diagnosed with cancer. The Active Together programme runs alongside existing diagnostic and treatment pathways to prepare patients for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. The programme combines physical activity, nutritional support and psychological support alongside the clinical pathway. This is a funded 5 year programme with staged implementation into all relevant cancer pathways. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs, which may be delivered within locality or selected facilities closest to the patient’s home or may be a self-management pathway. The overall purpose of this role is to lead, establish, deliver and sustain the success of the psychological element of the Active Together Programme. The Psychologist will ensure effectiveness and sustainability of the psychological service, delivering evidence-based practice and developing services within the Trust.
