Job Overview
If successful, you will be involved with a wide multi‑disciplinary team caring for older adult inpatients. It is essential that you are interested in and excited at the opportunity to work with older people – training will be provided. You will care for your patients holistically, involving carers and relatives, and work alongside medical and therapies staff. You will adapt quickly to changing situations and provide high‑quality care to your patients. We are keen to have staff who are passionate about older patient care and who are keen to be actively involved in service development.
Main Duties
- Responsible for the assessment, implementation and evaluation of care plans for individual service users on an Older Adult Inpatient Ward.
- Work as a key worker for individual patient needs.
- Assist the team in setting and maintaining high standards of care and evaluating quality outcomes.
- Participate in a range of assessments provided during the 24‑hour period.
- Maintain good standards of communication, both oral and written, in accordance with NMC guidelines.
- Report to the nurse in charge (or nominated deputy) any circumstances that may affect the delivery of care and/or efficient running of the ward.
- Work with all staff to adhere to Trust Policies and Procedures.
