Occupational Therapist Reablment

Overview

Occupational Therapist – Reablement Service in Chippenham. Temporary ongoing contract with Service Care Solutions on behalf of a Local Authority.

Responsibilities

• Undertake Occupational Therapy assessments within the community and reablement settings.
• Support adults to maximise independence and achieve personal goals.
• Recommend appropriate equipment, adaptations, and moving and handling solutions.
• Develop and review reablement plans to promote safe and effective discharge from hospital and prevent admissions.
• Work closely with multidisciplinary teams, carers, and community services to deliver positive outcomes.

Qualifications and Skills

• Degree or Diploma in Occupational Therapy.
• HCPC Registration.
• Previous experience within Reablement, Community OT, Adult Social Care, or Intermediate Care services.
• Strong assessment and care planning skills.
• Full UK Driving Licence and access to a vehicle preferred.

Details

• Rate of Pay: £36.00 per hour Umbrella; £28.78 per hour PAYE.
• Contract: Temporary Ongoing.
• Location: Chippenham, Local Authority client.
• Hours: Full-Time.
