Job Description:
Jobtailor is seeking a dynamic Social Worker to join the Social Work Hospital Discharge Team (West). You will work with adults with physical and/or mental health needs, their families and carers, promoting a person-centred approach to discharge planning in hospital settings.
In a multidisciplinary team you will undertake personal outcome focused assessments, complete risk assessments and risk management plans, and help ensure safe, timely discharges while travelling across Fife as required.
