Overview
ICS is a multi-disciplinary and multi-agency team operated jointly between Walsall Health Care NHS Trust and Walsall Council Adult Social Care. We are a dynamic, welcoming team working with adults, their carers and families. The role entails supporting individuals to maximise their full potential and promote their independence following an acute episode of illness or hospital admission. The team supports with hospital discharge, rehabilitation, crisis intervention and preventing hospital admissions. We are committed to working in collaboration with partner agencies to achieve positive outcomes for individuals.
We adopt a strength-based approach in our practice and outcome-focused support planning.
Responsibilities
The social worker supports with hospital discharge, rehabilitation and crisis intervention, and contributes to preventing unnecessary hospital admissions. The role includes undertaking statutory duties in line with the Care Act and Mental Capacity Act, and performing duties expected of social workers in line with the Professional Capabilities Framework.
Qualifications and requirements
Applicants must be Social Work England registered and have experience safeguarding adults. The role requires the ability to carry out statutory duties as dictated by the Care Act and Mental Capacity Act and to work within the Professional Capabilities Framework.

ICS is a seven-day service across 365 days a year, and successful candidates will be required to work weekends and bank holidays on a rotational basis.
