Overview

Adults Social Worker – Contract role with our client. A 2-week contract to join the Adult Early Support Team (AEST) in Manchester. This opportunity involves working within a dynamic, multidisciplinary team to promote independence and deliver preventative social care services to Manchester residents.

Responsibilities

  • Providing strengths-based, person-centred assessments for adults facing complex challenges
  • Developing and implementing tailored support plans to promote independence and prevent escalation of needs
  • Collaborating effectively with on-site partners including social care teams, police, mental health professionals, occupational therapists, and voluntary organisations
  • Acting as a key point of contact in the early support pathway, ensuring timely and effective interventions
  • Monitoring and reviewing support plans to ensure positive outcomes for service users
  • Advocating for vulnerable adults and championing their rights within a multidisciplinary framework
  • Maintaining accurate case records and ensuring compliance with data protection and safeguarding policies

Candidate Requirements

  • Minimum of 3 years’ experience working in a social care setting supporting adults
  • Strong collaboration skills to work effectively with diverse integrated teams
  • Relevant professional qualifications in social work as per JDPS
  • Clear references covering the last 3 years of employment
  • Ability to thrive in a fast-paced, ever-changing environment
  • Clear understanding of safeguarding, mental health, and adult support frameworks
