We are recruiting Band 5 Community Staff Nurses to join the District Nursing teams within CNWL Community Services in Hillingdon.

Key Responsibilities
n
As a Community Staff Nurse, you will work both independently and within a multidisciplinary team to deliver safe, effective, and patient-centred care within the community.
n
You will develop skills including:
n
- n
- n
Autonomous community nursing practice
- n
Diabetic management, blood glucose monitoring, and insulin administration
- n
Tissue viability and wound care, including:
n
- n
- n
Compression therapy
- n
Doppler assessments
- n
VAC dressings
- n
Pressure ulcer management
- n
Surgical wound care
n
n
n
n
n
- n
- n
Bladder and bowel care, including catheterisation and continence assessments
- n
Intravenous medication administration and PICC/midline care
- n
Palliative and end-of-life care, including symptom management and syringe drivers
- n
Management of specialist drains including nephrostomies and chest drains
n
n
n
n
n
n
n
n
You will also:

Maintain accurate clinical documentation and patient records
- n
Support patients, carers, and families with education and ongoing care needs
- n
Work collaboratively with specialist teams and wider community services
- n
Contribute to safe discharge planning and hospital admission avoidance
n

What We’re Looking ForEssential

- n
Registered General Nurse (RGN) with current NMC registration
- n
Excellent written and verbal communication skills
- n
Ability to work effectively within multidisciplinary teams
- n
Strong clinical and organisational skills
- n
Sound IT skills and accurate record keeping
- n
Understanding of risk management within community settings
n

Desirable

Evidence of relevant CPD

Previous experience within community or district nursing
