The role focuses on rapid assessment and delivering care in patients’ own homes to prevent hospital admissions, with work across Virtual Wards, Urgent Community Response and short-term rehabilitation.
You will register with HCPC, perform holistic assessments, coordinate with multidisciplinary teams, and contribute to service development.
